paper in Journal of Cardiovascular Electrophysiology Upon publication of this paper in April 2004, the authors received PDF copies of the published version, and we were free to distribute single copies of that PDF file to anyone who wanted one, but we were not permitted to post any version of the paper on the Web. From time to time after publication, the authors asked Journal of Cardiovascular Electrophysiology to relax this restriction. Our requests were denied until May 2006, with no clear explanation of what rules were being applied. Now we have permission to post any of our own versions of the paper, but not the PDF of the paper as it was published. To see the paper exactly as it was published, please communicate with JCE or with one of the authors to obtain a copy of that PDF file. Numerous rhetorical, grammatical, and typographic errors were introduced in the JCE editing process, and a few of them are listed here. This file shows the paper approximately as it was submitted, with a few sentences slightly improved and with scattered commentary (by me, and not necessarily representing the views of the other authors) indicating how the paper might be revised if it were to be re-done today.
